PostgreSQL MVCC: snapshots, xmin/xmax, visibility

Multiversion concurrency as a data structure: xmin/xmax, a snapshot as a set of numbers plus visibility rules, isolation levels, clog, and hint bits. This is where UPDATE = DELETE + INSERT finally makes sense.
